A Study on the Antegrade Pyelography


Abstract
Conclusions resulting from this clinico-pathological study on antegrade pyelography are as follows:
1. The patient was laid in the lateral position with a pillow under normal flank, and the diseased kidney uppermost. A line was drawn between tip of 12th rib vertically and the outer edge of vertebral longitudinal muscles; two fingers breadth caudal to the end of this line, punctures were made. The needle was inserted towards the contra lateral nipple.
2. In 57 kidneys which were not visualized by 1. V. P. and unsuitable for retrograde pyelography, antegrade pyelogram revealed pyonephrotic and hydronephrotic changes in 68. 7 and 31. 3% respectively; pyelectasis was more prominent in the latter than in the former.
3. In pyonephrosis and hydroneprosis the pyelogram was found to be enlarged by antegrade pyelography while the renal parenchymal image was shown to be decreased by renal scanning. Furthermore it was found that the more the pyelogram was enlarged the less viable parenchymal tissue remained.
4. The A. F. B. detection rate was more high in the pelvic urine than in the bladder urine. The urine from advanced renal tuberculosis and marked hydronephrosis was found to be more hyposthenuric or isosthenuric.
5. No noticeable pathological changes were found as a result of puncture in kidneys removed surgically.
6. Of the total cases that underwent antegrade pyelography, renal tuberculosis was found in 7¨Ï.9% and hydronephrosis in 29.1%.
7. In cases that underwent antegrade pyelography it was found generally that the renal parenchymal
loss was prominent while the total kidney weight and pyelogram was markedly increased.
In summary antegrade pyelography was found to be relatively safe and easy and considered highly valuable as a method of diagnostic procedure in advanced renal diseases which were not visualized by I. V. P. and unsuitable for retrograde pyelography.
